HOW TO SUBMIT A FUNERAL CLAIMWe understand that at a time like this, you need all the support you can get. That’s why we aim to process all valid claims as quickly as possible.
THESE ARE THE STEPS to follow when claiming from your benefit:
1. REPORT THE DEATHIf the death has not been reported by the executor, bank, trust company, funeral parlour, etc., please report thedeath to your nearest Home Affairs office that will also provide you with the official death certificate.
2. GATHER AND COMPLETE THE REQUIRED DOCUMENTSPlease provide us with the following documents to support your claim:
- Completed Funeral Claim Form (included in the Claims pack or available as a downloadable form at www.iwyze.co.za)
- Certified copies:
- Identity document (ID), (green bar-coded or smart card ID) of the:
- beneficiary
- claimant (if different to beneficiary)
- deceased
- Death certificate (provided by the Department of Home Affairs)
- Consent letter (provided by us)
- Copies of the following completed forms (provided by the Department of Home Affairs):
- DHA 1663 (Notice of Death)
- DHA 14 (Burial Order)
- Police report (required in any event where the insured person died from unnatural causes)
